Lab management software built for a US hospital network doesn't automatically fit how a diagnostic lab in Delhi, Mumbai, or a tier-2 city actually operates. Patient volumes, payment patterns, referral-doctor relationships, and communication habits are genuinely different — and the software you pick should reflect that, not force your workflow to match someone else's assumptions.

1. NABL and accreditation readiness

If your lab is pursuing or maintaining NABL (National Accreditation Board for Testing and Calibration Laboratories) accreditation, your software needs to support the documentation trail accreditation actually checks: QC records with corrective actions attached, full audit logs of who accessed or modified a result and when, and traceable equipment/calibration records. Retrofitting this after the fact is far more painful than choosing software that has it from day one.

2. GST-compliant billing, not just "billing"

Generic invoicing modules built for other markets often don't handle GST correctly out of the box — proper GSTIN capture, correct tax treatment for diagnostic services, and GST-compliant invoice formatting. Diagnostic chains with multiple collection centers feeding into a central processing lab are extremely common in India, even for mid-sized operators. If you're likely to add a second location eventually — and most labs are — check now whether the software handles branch-level access control, sample routing between locations, and consolidated-plus-branch-level reporting, rather than assuming you can bolt this on later.

A large share of testing volume in the Indian diagnostics market comes through doctor referrals with commission-sharing arrangements. Software that can track referral sources and calculate commission automatically per test or package — rather than requiring a manual spreadsheet reconciliation every month — saves real administrative time and reduces disputes.

6. Walk-in-heavy front desks need speed, not friction

Many Indian labs handle a high proportion of walk-in patients rather than pre-scheduled appointments. Registration, sample collection, and payment collection need to be fast at the front desk — barcode-based sample identification and a registration flow that doesn't require excessive mandatory fields both matter more here than in appointment-only models.

7. Data hosting and residency

Ask directly where patient data is hosted and what jurisdiction governs it. This affects both your own compliance obligations and how comfortable hospital and corporate clients will be signing on as referral partners.

The best lab software for an Indian diagnostic lab isn't necessarily the one with the longest feature list — it's the one whose defaults already match how labs here actually run, so you're not spending the first six months customizing around gaps.
